ADDA has changed the submission requirements for all new Curriculum Certification Packages. Effective April 01, 2005, all Curriculum Certification Packages are to be submitted on electronic documents and furnished directly to ADDA on Compact Disk. This requirement will allow schools, institutions, and agencies the opportunity to reduce cost and allow easy access to information for verification and inspection. Please download the zip file which contains paperwork, instructions, all folder, forms, and tabbing that conforms to the prior submitted binders. All forms must be submitted as PDF Documents.
