ALL CURRICULUM CERTIFICATION RENEWALS MUST BE
SUBMITTED ELECTRONICALLY via EMAIL or on CD

NOTICE OF PRICE CHANGE
Due to the increase in Operational Cost the ADDA Board of Directors
implemented a price increase for Program Certification starting January 01, 2010.
ADDA has neglected any increases in our curriculum operational process since 2004.
Existing Programs Recertifying will be increased only by $35.00 at all levels.
All New Programs Certifying will be Increased by $75.00 at all levels.  

Please read the instructions carefully.
This Package Contains the Application & Renewal Forms
There are specific requirements for attachment file names.
Refer to the top of page 1 of the Renewal Form Section
for specific directions on file names & submission.
